Weight is a solid-disc estimate from the outside diameter and the minimum thickness at 0.2836 lb/in³, and ignores the raised face, so it runs slightly light against a real forging. End thrust is the rated pressure acting on the raised face area; it is not a bolt-load calculation, which must also cover gasket seating.

4 holes of 0.625 in diameter on a 3.12 in bolt circle, taking 1/2 in bolts. B16.5 holes are 1/8 in larger than the nominal bolt. They straddle the vertical and horizontal centrelines rather than sitting on them, so hole spacing round the circle is 2.45 in.
What pressure is an NPS 1 Class 150 flange good for?
